What is Protective Supervision?
Protective supervision is an IHSS service category for recipients who are non-self-directing due to a mental impairment or mental illness. It provides observation and supervision to prevent injury, harm, or accidents. Unlike other IHSS services, protective supervision can authorize up to 24 hours per day of care.

The SOC 821 Doctor Form
To qualify for protective supervision, your doctor must complete the SOC 821 (Physician's Report for IHSS Protective Supervision). This form asks the doctor to certify that the recipient is non-self-directing and at risk of injury without constant supervision.
SOC 825: Coverage Plan
The SOC 825 is used to document the plan for protective supervision coverage, including who will provide supervision and during what hours.
Can My Doctor Charge Me for IHSS Forms?
No. Under California Health and Safety Code Section 123114, your doctor cannot charge you for completing forms required for government benefit programs. This includes the SOC 821, SOC 825, and all other IHSS-related medical forms.
